Assured Assistance is excited to announce the addition of Case Management Services to our growing range of senior care and support services.

As families navigate the challenges that often come with aging, changing health needs, hospitalizations, new diagnoses, and coordinating multiple healthcare providers, we recognize that caregiving is sometimes only one piece of the puzzle. Our new Case Management Services are designed to help families better understand and coordinate the many moving parts of their loved one’s care.

Leading this new service is Kristin Galle, RN, our Case Manager. With her over 30 years of nursing background, Kristin will work closely with clients, families, caregivers, physicians, healthcare professionals, facilities, and community resources to help ensure that everyone involved in a client’s care is working toward the same goals.

What Can Case Management Provide?

Our Case Management Services can provide support with:

• Coordinating care between families, caregivers, physicians, and other healthcare professionals
• Helping families navigate changes in their loved one’s health or care needs
• Assisting with hospital and rehabilitation discharges
• Attending medical appointments when appropriate and helping families understand recommendations
• Communicating important updates between members of the care team
• Connecting families with appropriate community resources and services
• Reviewing medications and helping identify questions or concerns to discuss with healthcare professionals
• Monitoring changes in condition and communicating concerns to the appropriate members of the care team
• Helping families develop and adjust care plans as needs change
• Advocating for clients and helping ensure their needs and wishes are communicated
• Providing support during major healthcare transitions
• Assisting families with navigating chronic illness, increasing care needs, and end-of-life planning and support

We are proud to have Kristin Galle, RN leading our Case Management Services. Kristin brings more than 30 years of nursing experience, compassion, and commitment to helping seniors and their families navigate the complex healthcare system.  Kristin is committed to providing individualized, compassionate care by meeting each person where they are and developing personalized care plans that reflect their unique needs, goals, and preferences. As a dedicated advocate, she prioritizes each client's well-being, empowering them to maintain the highest possible quality of life while aging safely in the comfort of their own environment. Outside of work, Kristin enjoys spending time outdoors with her husband, appreciating the peace, beauty, and rejuvenation that nature provides.
